Power of Attorney (POA) :   This legal document is used to authorize another person to make legal, financial, or medical decisions. Inmates often need a POA to allow a trusted individual to manage their affairs while they are incarcerated.   

Affidavits:   An affidavit is a written statement confirmed by oath or affirmation, used as evidence in court. Inmates may need to provide affidavits for various legal proceedings or to assert facts in civil or family law cases.​​   

Consent Forms for Legal Representation:  Authorizes an attorney or legal representative to act on behalf of an individual in legal matters, confirming consent and granting authority for decisions and actions related to the case.

Domestic Partnership:  Allows an inmate to register his/her domestic partnership while incarcerated. 

Name Change Through Court:  Supports the legal process for inmates seeking to change their name through the court system.

Guardianship:  Needed to formally appoint a guardian for minor children or for individuals unable to manage their own affairs.

Visitation Forms for Minors:  Grants permission for a minor to visit a designated incarcerated individual, ensuring legal consent for the child's safety and well-being.

Parental Permission to Travel:  Required for a minor to travel with one parent or a guardian, providing legal proof of consent.

Child Passport Forms:  Needed for a child's passport application when a parent is incarcerated.

Vehicle Transfer Title:  Required to legally sell or transfer ownership of a vehicle while the owner is incarcerated.

Wills and Estate Planning Notary:  Allows inmates to document and formalize their wishes regarding property, assets, and inheritance matters.
